To make an appointment, contact any one of our offices directly between 9AM-5PM, Monday through Friday. Prior to calling, you may want to formulate a list of questions you may want us to answer during your call.
Please check with your insurance provider to determine if you need a referral from your primary care physician, and/or pre-authorization for your appointment.
We suggest you plan to come in early for your appointment, since there are usually forms to fill out, questions to be answered and insurance cards and other forms of ID that may need to be copied. This information is necessary to manage your billing and process your paperwork. This is all part your medical record with Advanced Urology Centers of New York AUCNY.
When you call to make your appointment, we may mail you some forms in advance, and ask you to bring in test results, films or paperwork from other physicians, hospitals, surgical centers or diagnostic testing you had done prior to your appointment.
We’ll ask you what medications you may be taking, and what over-the-counter products you may be using for various purposes (like vitamins, supplements, herbs, etc.). If you’re allergic to anything it will also be important for us to know that information.
If you are under 18, you must be accompanied by a parent or guardian.
